I want an online form allowing people to submit their membership application on line. I'd be happiest with a result that will take the existing CCE membership form (in PDF) and transform it into a PDF with data entry fields. The form would then be emailed to the CCE office as a completed PDF.

Hey Jessie. There is an online enrollment form. It's just a bit clunky to get to. I've been working on a "work instruction" doc with screen caps to pubish to html or PDF to share with folks. I figured if I posted this it would "encourage" me to make finishing this doc be more of a priority. :)
